Tryout Consent Form

Vanderbilt Men’s Basketball will have tryouts on Wednesday, December 7, at 7 p.m. in Memorial Gym. Please come dressed ready to participate in drills and games.
You must bring your Vanderbilt ID card and the filled out release of liability and medical consent forms and tryout pre-participation physical evaluation form. All must have proper signatures.
The physical form must be completed by a physician or nurse practitioner and participants must have a sickle test drawn and results in order to participate. If you have had one done previously you must have documentation from physician. Sickle cell tests may take anywhere from 48-72 hours to get results from your physician/medical professional.
